• We will always try to schedule your appointments promptly.

• Please make sure your primary care physician provides us with a referral before your appointment.
  

• Call within 24 hours to notify us if you can not make your scheduled appointment.
  

• Arrive 15 minutes before your scheduled appointment time in order to properly check in and prepare for your visit.

• Co-payment is expected at the time of visit.

• We do not refill medications routinely by phone or fax.   Please ask for your medication refills at the time of your visit.

• We will always work closely with your primary care physician in order to provide the best and most appropriate care for you.
