Driving Directions

    * From the West
      o Take the Kennedy Expressway (I 90). If coming from the south, exit at Diversey eastbound. If coming from the north, exit at California Avenue and turn left onto Diversey Avenue eastbound.
      o Drive east on Diversey Avenue to Sheridan Road (same as Lakeview) and turn left.
      o The parking lot is on the right on Sheridan road.

    * From the North
      o Exit Lake Shore Drive at Belmont Ave.
      o Turn right onto Belmont avenue.
      o Take the first left onto Sheridan road.
      o The parking lot is on the left on Sheridan road.

    * From the South
      o Exit Lake Shore Drive at Fullerton Ave and drive west
      o Turn right at Cannon Drive
      o At split, continue straight onto Sheridan road.
      o The parking lot is immediately on the right on Sheridan road.


Public Transportation

    * CTA Bus Routes - The #76, #151, and #156 bus routes stop directly in front of the clinic at the corner of Diversey and Sheridan. From the west take the #76 (Diversey line).  From north or south take the #151 (Sheridan line) line.  And from the Loop use the #156 (Lasalle) line.

    * Rapid Transit - The Red, Brown, and Purple "el" lines stop at Belmont avenue. From there take either the #151 or #156 bus to the clinic. The Brown and Purple lines also stop at Diversey Avenue. From there, take the #76 Diversey bus east to Sheridan Road. 

Parking

Patient parking is available across the street on Sheridan road. Discounted parking for patients is available.


Entrance

Please enter the building via Diversey avenue. The clinic is located on floor 1, which can be reached by taking the elevator from the ground floor at the entrance.
